Integrative Harm Reduction Psychotherapy & Psychedelic-Assisted Therapy for Problematic Substance Use & Other Addictive Behaviors: A Three-Day Clinical Training
Led by Andrew Tatarsky, PhD & Casey Paleos, MD
December 11–13, 2026 · InnerMost · Gramercy, NYC
What if the most effective entry point into psychedelic-assisted therapy for substance use isn’t abstinence, but honest, compassionate curiosity about why someone uses? This November, InnerMost, Journey Clinical, and MAPS are co-hosting a three-day clinical intensive bringing together two of the field’s most experienced practitioners.
Andrew Tatarsky, PhD is a clinical psychologist, educator, and internationally recognized leader in the treatment of substance use and addictive behavior. Over more than four decades of clinical practice spanning 20 countries, he developed Integrative Harm Reduction Psychotherapy—a compassionate, evidence-informed framework that integrates relational psychodynamic therapy, CBT, mindfulness, and trauma-informed care within a harm reduction frame. He is the founder of the Center for Optimal Living in New York City, author of Harm Reduction Psychotherapy: A New Treatment for Drug and Alcohol Problems, and a Professor of Professional Practice for the Harm Reduction Psychotherapy Certificate Program at the New School for Social Research. His clinical intensives have a history of selling out.
Casey Paleos, MD is a board-certified psychiatrist, educator, researcher, and co-founder and Chief Medical Officer of InnerMost. He served as Principal Investigator and Certified Therapy Supervisor on MAPS’ Phase II and Phase III MDMA-assisted therapy trials and as a Study Therapist on NYU’s landmark psilocybin cancer anxiety study, and has trained the next generation of psychedelic therapists through organizations including MAPS and Fluence.
This training builds directly on that research legacy—bringing the clinical rigor of the MAPS trials into dialogue with four decades of harm reduction psychotherapy practice. For clinicians working at the intersection of psychedelic-assisted therapy and substance use, this training offers something the field has been missing: a rigorous, relational framework for working with patients who carry both a psychedelic therapy indication and a complex relationship with substances, without requiring abstinence as a precondition for care.
Across three days, participants will: Engage with the theoretical foundations and clinical applications of IHRP Examine how harm reduction and psychedelic-assisted therapy share a philosophical root—and what that means in practice. Build skills through demonstration therapy sessions and structured real play Join a facilitated think tank on the genuinely unanswered clinical questions at this intersection
